If you have recovered or been vaccinated with documentation that is accepted by Israel, the Israeli authorities do not require you to take a PCR test prior to departure. However, many destination countries do require a negative Covid for you to be allowed entry. Please see here for current information on entering the US.
All travelers departing Israel (regardless of age) who are not recognized by Israel as vaccinated/ recovered require a negative PCR test within 72 hours of departure. Results must be in English and must show your passport number. See here for a list of Corona testing locations.
Only ticketed passengers, with a flight in the next four hours are permitted to enter the airport terminal.
Additionally, Israeli citizens and toshavei keva (permanent residents) 16 years of age or older must fill out this form within 24 hours of departing Israel. This form is not a health declaration. Rather, it requires passengers to affirm that they will not travel to countries on the Red list. See the entire list here.
